What is the ACT 24 form for TB?

Volunteer Coaches must complete the Arrest/Conviction Report and Certification Form (ACT 24), and have up-to-date TB test results prior to beginning coaching, regardless of amount of time spent with students.

Can you volunteer with CBSD?

If you are being compensated through CBSD by paycheck, parent fund or student fund; you are NOT a Volunteer. The Volunteer Tracker will allow you to sign-up for volunteer opportunities specific to your building and/or classroom of choice.

Reporting Absences

Absences should be reported to the teacher or school office within three (3) days of the absence, listing the date and reason for the absence. Excuse notes may be submitted using the printable or online forms below.

Attendance for Extra-Curricular Activities

Students who are absent from school are not allowed to participate in any extracurricular activity until he/she has been in attendance for an entire school day.

College Visitation

The student must turn a College Visitation Form completed with parent/guardian, teachers, and student signature to his/her school counselor at least one (1) day prior to the visitation date. Forms are available in the Students Services Office at CHS/CMS Hill Campus.

Job Shadow

The student is responsible in arranging their own transportation to and from site.
