To enroll, patients must accept a patient portal invitation sent to their personal email address. If you did not provide an email address during your most recent visit or you can't find your email invitation, call 706.509.3236. To receive an invitation, we will ask a series of ​questions to verify identity.

How is my Patient Portal determined?

Your patient portal is determined by where you received care. Primary Care or Urgent Care - patients who have visited an urgent care location or a Floyd family medicine provider (primary care office)​

Can I access my child's school records if I don't have internet?

If you do not have internet access, the public library has computers open to access your child's records. Please keep your passwords confidential so only you can access the information. The Grades and Attendance tab will allow you to check assignments and scores by clicking on the blue percentage across from a class.
