Student records follow the student from school to school at the DOE. Your last school, or current school, is always the best place to start when looking for student records, including transcripts. You can request your records at any time using the instructions in the following sections. We do not provide duplicate diplomas.
If your former high school has closed, another school now has your transcript and can provide you with a copy. use this tool to find out which school.
This form asks for student information, school information, and a signature from the student or parent/guardian. You can request records electronically, by mail, or in-person. If you are unsure how to submit your request, call the school first.
You may need to provide records of graduation, attendance, or enrollment as documentation to other agencies and programs. Follow the directions above to request these records. If you need your records sent directly to another agency or program, you will need to consent to releasing the records. Contact your school for assistance with your request.
Polk County Public Schools main district offices no longer accepts walk-ins for any student records requests. If you would like to request your student transcripts, please see the appropriate option below if you are a current student or a former student.
Please remember that current student records have another set of statutory requirements. Requests for the records of current students can be made at the student’s school site.

The student’s temporary records include all school-related student information not contained in the permanent record. Student records may include both paper and electronic records. CPS can provide former students with a copy of a high school transcript, verification of a graduation date from high school, a copy of a elementary school transcript, and an elementary registration card or immunization records. Current CPS students should see their school counselor to request a copy of their transcript.
