Each year, families are required by the state of Illinois to turn in medical forms for each student enrolled. There are three categories of forms, which are outlined below. Please carefully read this information to determine which forms pertain to your child (ren).
The Medical Update Form and the Emergency Contact Form will be completed online through the Portal only and they need to be updated annually. Families with students in Grades 5 - 8 will also have a Concussion Information Acknowledgement to complete as well.
