If your child attends Monroe Township Middle School, Applegarth Elementary School, Barclay Brook Elementary School, Brookside Elementary School, Mill Lake Elementary School, Oak Tree Elementary School or Woodland Elementary School please bring the completed form and identification to the main office of that school.
Contact the Main Office of the school your child attends. If you have more than one student in the district, choose whichever school is most convenient for you.Once documents have been viewed and accepted, parents/guardians will create a Parent Portal account linked to their personal email.
