Student transportation forms are only needed for use to and from specific MSM events. Please consult with the director of your event, field trip or sport before using these forms.
Each MSM student athlete is also required to fill out a physical form. Please print the physical form, complete both sides and return to MSM Assistant Athletic Director Leroy Cantrell. A student is not allowed to participate (practice or play) until this physical form is on file with the school.
