Your portal enrollment card and medical record number are required to enroll in the Stamford Hospital Patient Portal. If you already have your portal enrollment card and your medical record number, you can self-enroll. If you do not have your portal enrollment card and medical record number, contact the Medical Records Department at (203) 276-7034.

To find out if your physician is part of the Stamford Health Medical Group, go to our Find a Doctor search. Get convenient, secure access to your health information from your smartphone using Healow – a free app available through the Apple Store and Google Play. Use the practice code ABCIBA to set up your app.

What is the cures act for Stamford Health?

The Cures Act ensures patients have access to their electronic medical records via the patient portal without delay. All completed provider notes, laboratory tests and diagnostic tests will be available on the portal for both historical and recent visits, this may also include previously unavailable sensitive results such as HIV, STD, genetic testing, etc. Please make sure only appropriate individuals have access to your portal through the proxy access. If you have any questions about accessing the patient portal, please contact the Health Information Management Department at 203.276.7034.
